The full health form application is an electronic version of the Common Application Form and allows advisers and consumers to work together to fill in the form before using the collected data to pre-populate the existing Common Application Form. What are the advantages?Fill out the Common Quotation Request Form online. The Annuity Health Form automatically populates the Common Quotation Request Form PDF. Share Common Quotation Request Forms with your clients. Clients can be given secure access to the appropriate sections of the form so they can fill out the details. Send your Common Quotation Request Forms securely to multiple annuity providers. You have the choice of sending online forms or optionally uploading existing PDFs. Excerts from "How to use the health form"Create a formTo begin, click the 'Create new Health Form' link on the Home page. You can now enter the basic client details.
Form navigationOnce saved, you will be able to navigate the various sections of the form using the navigation tree on the left. Each section corresponds to a section within the Common Quotation Form PDF. You can fill out the sections of the form in any order, make sure you hit the Save button to save your progress as you go along. You can come back to a partially completed form at any time. Adding conditionsMultiple conditions can be added to the form for both annuitant and dependant. Click the 'Add new condition...' link in the navigation tree under Annuitant or Dependant. You will be given the option to choose a medical condition. Once the condition is added, the appropriate questions will be displayed for that condition.
Preview the Common Quotation PDFAt any time during the process of filling out the form, you can preview how the final PDF will look by clicking the 'Preview PDF' link in the top-right of the screen. The PDF will be populated with all the information you have entered and all appropriate additional pages will be added for multiple conditions etc.
Sending a form to annuity providersOnce you have completed a form, you can send it to one or more annuity providers by clicking the 'Send to providers...' action link within the form screen or on the Health Forms summary screen. Select the providers you want the form to be sent to and enter any additional information in the comments section. Clicking Submit will then notify the chosen providers that the form is available for download. ![]() |
